Apply full dialogue mechanics (L.3.2.c): quotation marks, commas, capitals, new paragraphs

Use the four dialogue mechanics rules consistently: (1) quotation marks hug the spoken words; (2) comma separates tag from quote; (3) capital first word of the quote; (4) end punctuation goes inside the closing quotation mark (US convention). New paragraph for each new speaker.
